I'm watching this thread because I too am having this issue. Apple Watch series 3 with LTE. Latest update. This is my first Apple Watch so I thought maybe I was doing something wrong and just kinda forgot about it (Watch was a Christmas gift so I'm still learning how it all works) until I seen this thread here. I too do not have anything scheduled in my calendar and I have it set to remind 3 times a day. Some days I've gotten none.
 
It's somewhat ironic if it is getting affected by calendar notifications, because the easiest workaround is just setting recurring reminders in the calendar app manually.
 
I too have had this problem since 4.0. I sent in the info to engineers back then..and still no fix or even acknowledgement. it's pretty crazy to have something that seems so basic and easy to be so badly broken.
 
I hardly ever get them. I might get maybe two notifications per week. What's also weird is that sometimes, I get notifications to breathe at 2 or 3am (discovered it in my notifications after I woke up). My watch is set to the correct region and time zone. Also, my calendar is pretty much empty so it's not that that could be affecting my notifications.

Like others on here, I find it frustrating and I hope Apple can sort this out soon.

Silly answer... This is a bug. I clearly often meet these requirements being an IT professional... Also, the whole value of the breathe app is to nudge you to take some time-out... so I dont get it or buy it... its poor design or a bug. Worked better in 3.x

Also if I set my reminders to 10 a day, then I should get 10 a day not 1 or 2 a week. :)

My watch was a Christmas gift so I haven't had it that long yet, however I too am not getting breathe reminders. Every great once in awhile one will pop up.... seemingly random. I had one pop up WHILE I was doing a workout but they are still very few and far between. So I don't particularly buy the idea that they won't trigger when doing a workout or other activities.
 
Oh good I’m not the only one then! This ugh the I hadn’t been getting them, but considering I’ve had to reinstall iOS 11 on all my devices I’m not surprised Watch Os4 is buggy and also needs a fresh install lol...

Seems to be the Apple way now? Still think my Watch is one of the best things Apple has made in a lllooonnngggg time, perhaps since the iPhone 4?

I rate my all time great Apple products as my watch, iPhone 4 and my 3rd gen fat boy iPod nano I still have and it still works :)

Oh.... was this in the release notes? Does seem like a silly excuse like the battery throttling issue!
If they want to change things just TELL PEOPLE!! Then we won’t waste our time trying to fix it.....
 
I was able to speak with a Sr Advisor @ Apple support. After going through all their checks and balances to make sure the settings were correct and that all troubleshooting steps had already been done, they finally told me that this is a known issue and that they are currently “Collecting and Logging data” about the issue from anybody who reports it. They are not doing anything yet to resolve the issue, only logging info to determine which engineering team has to fix the issue. I let them know about the 1000s of users that are complaining about it on the different industry forums, including their own Apple forums, and I supplied links to every thread I could find about this issue. They said the issue is on their end and there is nothing I can do to resolve it since everything is already set properly. Bottom line, you got call into support to report it or do what I did, start a support ticket from Twitter. Go to Apple Support’s Twitter page and post a message. They will have you contact them via a DM, then it’s create a ticket from there and get your info logged. The Twitter tech did try to pawn me off and said this is a new design from Apple for the Breathe app and that you had to be stationary for a while before it would remind you. Once I told them that counters everything that Apple said the Breathe app was for in the beginning, they backed down. Next thing you know, I’m getting sent off to a Sr Advisor to review. Don’t settle for some cheesy response on Twitter and you should get through to the next level.

Struggling for breath here ha, wondered what had happened to the breathe notifications, thought I’d turned something off. Has there been a resolution?

I wonder how they'd explain the fact my watch had a breathe reminder while it was on the charger, 50 feet away from me?
 
I guess support's response explains why most of my breathe reminders (if I'm lucky enough to get ANY) happen in the middle of the night, when I'm SLEEPING????? I don't understand why this has not been fixed yet. I'm not buying the answer from support, because even when I'm sedentary during the day and have nothing on my calendar, I don't get breathe reminders. I'm lucky if I get 2-3 per week.
 
I just got off of an hour long Online Chat with support, while updating both my watch (Series 2) and phone (6S) to the latest OS, and I shared with him all the forums I have read, including this one. Basically all he told me was, in regards to what Apple Engineering is working on, "I can see it progressing but its not out as yet, it should be via update but no time stated as yet" and I asked if there was a case or bug ID I could get for it, and he said "Oh no ids for the bug. Well not one that I can give. Its internal only, apologies, but there is a case for it".

So there, you have it....at least I have my Operating Systems' updated :)

I received 3 notification reminders yesterday, for the first time in forever, but none today.... :(
 
Sorry because this is out of this topic but maybe it could be related.
I use the Siri Watchface and I had my Photos synced so I got some Memories on my watch.

They suddenly stopped showing on the Watchface so I hard reset both of my devices and then I see them again but somehow they disappear later on.

So frustrating =(
