We need to boost the signal from the BT Broadband router.

A friend has suggested a Netgear signal increaser.

Is it difficult to set on up? Can it be used by the macbook and and iPhone 4?

The Netgear one on Amazon is around £45. Too much? Too little?

Airport is a router?

If it is a booster, does it need to be connected with cables to our existing router ? (This would present some physical obstacles).
 
I have used an Edimax EW-7416 in several people houses to do this from a BT-HomeHub. Works very well - just set it up (with an Ethernet cable first) then disconnect the cable and move it to a place half-way between your BT-HH and the weakest place in your house you want to cover.

Works well, cheap and powerful, but can be tricky to setup, although there is a Windows wizard.

I only say this as if the sister in law has one, it might be helpful to have continuity so we can compare faults/setup issues.
 
Quite possibly. I guess if your sister-on-law has one and it's doing exactly what you need it for then go for it.

Airport Express is cheaper than the Extreme.
