Nope. I haven't given the app much attention yet. I was just making a suggestion for those wanting to play SMB on their iPhones.
I personally would back it up to iTunes. That's the most guaranteed way. iCloud is not old enough for me to trust that things won't just disappear from there when pulled. But with your iTunes backup, it is your own personal file.

I downloaded this from app store onto my 4s. But can someone tell me how to make sure I dont loose it please? I looked in my app store and it's gone from the list so I know I can't reinstall it. Do I have to back up in iTunes or is iCloud enough? And in iCloud do I just slide the switch to the ON position in iCloud backup.

Plug it into your computer right click you iPhone > transfer purchases. Done its backed up.

This is one of the main downsides to the App Store model. Sure it's convenient for software developers to sell their wares, but if the software ticks off someone at Apple, or makes them fear legal issues (likely in this case) it's gone, with no recourse to install it.
Sure, there's jailbreaking, but it's unreliable. A good example is there's no way to jailbreak an iPad to running iOS 5 for the moment.
Apple really should allow side-loading of apps. They should make it a pain in the ass to do, so casual users don't do it, but it should be possible. The Palm Pre was locked down as well, but you could type in a code to allow side-loading (upupdowndownleftrightleftrightbastart), so the geeks were happy.
Of course they never will, but it'd be nice. I constantly feel at odds with myself because I love the platform, but hate the restrictions that come with it. It would be 20% cooler if iOS were truly open.

You'll have to jailbreak it. When you download the app from the store, Apple signs it with their key and your key. This is what allows you to run the app.
Since Apple won't sign it anymore, you can't use it on your device. You also can't use someone else's copy even though it's free, because it's signed with their key, not yours.
Jailbreaking is the only option you have, then you can install it using Cydia. Jailbreaking turns off code signing to allow any code to run. Read my previous post for my feelings about this. It's annoying!
