what is the device with the usb port at the top of the box? the thing with the prongs in the middle seems to be the power adapter

It's certainly not a UK adaptor. It's a pin short.

It's just a two part adaptor (same as the old iPhone had. They just put the two peices together in the box last time). It's very likely for Southern Europe, given the picture shows a Portugese website.

Only the US gets the tiny power adapter. All other countries, due to the varying electrical outlet configurations, get a small version of the original iPhone/iPod power adapter, with swappable outlet interfaces depending on the country. the power adapter shown (top with the USB port) is similar to the travel one. The middle thing slides into the side of the power adapter not seen.
