Here's how I make them in Photoshop CS4:
Size the logo, 75% works best for most images.
Cut out the logo (I use magic wand, there's about 50 ways to do it)
Open the background image (can be found in this thread or make your own)
Use the arrow to drag the cutout onto the background image
Position the logo how you want it.
Select the layer with the logo on it, right click and choose Blending Options...
Where it says "Blend Mode", choose Overlay
Save as JPG.
