Here's my full prognosis just to compare. iPhone 4 AT&T, iOS 4.1, jailbroken. Single alarm set, clocks changed correcty but the alarm shut off; however the alarm icon was still in the top menu bar when I checked it this morning.
 
Mine rolled back an hour so it was two hours behind. Rebooting fixed it.

That happened to me last night... at midnight, the time rolled back to 11:00 pm. After a brief "wtf?" moment, I just restarted my phone, and it was back to 12:00. Then at 2:00 am, the time automatically jumped up the hour to 3:00 am, no problems there.

Verizon, if that makes a difference. All of my Verizon phones have updated properly in the past, never had any issues...
 
Yeah, a simple restart fixed everything

I guess the real question is: how to restart your phone while sleeping? :D
I also know that a restart fixes anything, but when a person goes to sleep at midnight and has to get up at 7AM I find it unbelievable that he/she cannot rely on a $300+ piece of electronics to get a correct wake up call...
No excuses for Apple. And yes, I do have the latest update available for my old 3G (now discontinued and unsupported, shame on you Apple!)
 
I had no problems yesterday or today. I had to set my alarm for 4:00am and so it was important to get up. The alarm woke me up no problem. I'm on 4.3 by the way.
 
I am in Canada. My iPad on Rogers is 1 hour behind. My iPhone on Fido seemed unaffected by the issue, my alarm even went off at the correct time. Then I restarted the device and now it is also an hour behind.

i'm with fido and i'm still an hour back as of last night. kept the phone in manual time for the time being.
 
Does anyone know why this is so inconsistent?

My phone is fine.

My girlfriend's is not. She's not happy and I don't know how to fix hers.
 
Make sure your time zone is set correctly in both places it can be set.
  1. Settings->General->Date & Time
  2. Settings->Mail, Contacts, Calendars

Never had an issue with the DST on my iPhone and I'm still on 4.1. All my alarms went off as they should have.
 
I did some experimenting with this and found out that if I set my alarm as a one time, non-recurring alarm, it does indeed go off at the correct time. Now if I set the alarm to recurring for any amount of time, whether it be for 2 days, 3 or everyday, the alarm will go off one hour early. So as of right now it's rather I set the alarm an hour off, or set a new one everyday.

This is the same "solution" for me. I'm running 4.1 jailbroken iPhone. Tried rebooting, switching to airplane mode and back, deleted old alarms, added new ones, changed my time from auto update to my current time zone.... Nothing worked. I guess I'll just have to manually set it everyday.

same with me. jb on 4.1. played around with this for around 30 minutes. rebooted. airplane mode. changed date and time settings back and forth between automatic and manual. still no results. alarm goes off an hour early. im stuck having to have my 7:00 alarm set for 8:00 to correctly go off at 7. it would suck if some way or another my phone adjusted on its own and went off on the scheduled time

I have thought this very same thing. I too have mine set for 8am, cause I get up at 7. I guess that one day it goes off when it's "supposed" to I will be late and know that I can finally start using the alarm properly.

I keep hoping someone in the jailbreak community will figure some little something to install to fix it, but haven't heard of anything.
