Is my XR the world’s first one with swollen battery?

My XR is just 1-month old and the battery is swelling.... not sure if its too new to be eligible for battery replacement?

Doesnt need to be eligible for a new battery...just take it to Apple to swap the phone end of story.

How do you know that it’s swelling?

Just take it into Apple and they’ll replace the phone. Battery failures happen.
