I can guess what's happening.

You're on a call and when you go to hang up, the person at the other end beats you to it (maybe by fractions of a second).

Well, then the iPhone goes into the mode where you are now pressing the pause/play button.

Been trying to get Apple to delay that happening till at least 2 seconds after the call ends

Unless you change the settings, the same button on the EarPods/AirPods that hangs up a call also is the play/pause button for music. If the other party hangs up the call before you do, and you press the EarPod button or tap the AirPod to hang up the call, it reads the press/tap as the play button, and starts the music.

In case of something like headphones, that type of thing might potentially be in play (no pun intended). That said, it doesn't seem that something like headphones was mentioned by the OP.

If headphones aren't in use, I can't imagine how music can start just by hanging up a call, unless it was playing before the call and resumed afterwards.
 
I have this problem as well. Created an account here just to post. When I hang up a call via airpods my Music app or Pandora will intermittently start playing. If Pandora is "closed" (swept up / off) then it will be the Music app that starts playing. This happens only sporadically, and the music will continue playing even if I swipe up / off all the apps, because the Music app was never open. I have to swipe down from the top of the screen to get control center and pause that way. 35 minutes on the IT Chat with Apple only got me as far as unpairing my airpods, checking my phone for updates, and restarting it. Not a super critical issue but frustrating, and sometimes if my volume is up it's quite unexpected. Was visiting a client site one time and loud rap music started playing and I was embarrassed.

That’s not the case I use my phone regularly with the BT attached to the radio and when I touch nothing and just allow them to hang up it will start playing random songs Without being queue by the playlist

That seems to be somewhat different than what that was in reply to, as was also discussed in subsequent posts.
 
This happens without bluetooth earbuds. I know, because it happens all the time using wired headset, and using the disconnect icon on the actual phone (vs. the wired headset buttons). It doesn't matter how the call is disconnected, the music starts playing as soon as I hang up. It is very much a bug, since if you leave your earbuds connected, but take your earbuds out of your ears, you may not realize your phone is just playing music and draining battery life. Apples music app should not play anything if I don't open it and play it. That is a bug. They need to own their issues and fix.

These steps worked on iPhone 8 (software ver 13.4.1)

1 Open Music App on your iPhone.
2 Click on Library tab (located at Bottom Left).
2 Click on "Edit" button (located at Top Right).
3 Uncheck all items (such as playlist, artists etc).
4 Click Done (located at Top Right).
5 While you are still in the Music App, Click on Search tab (located at Bottom Right).
6 Clear all Recent search.
7 Close Music App

Discovering this thread made me so happy. All these years I thought i was the only one troubled by music playing automatically. The problem was first reported in 2015. It persists today. In the meanwhile I did try all the neem hakim cures discussed in the various responses. From 2015 till 2020!! Shall eagerly await the time when music does not play on its own. Would this happen only after a vaccine for covid19 is found?

Thanks for the chuckle. :) And yes, still happening. I've gone through all the supposed fixes, even went so far as to do a factory reset on the phone. No difference.
 
I know this thread is for iOS, but I was having the same problem on macOS. When I would hang up from a call made using a Bluetooth headset paired with both my iPhone and macOS (Catalina) desktop, iTunes would (almost always) fire up and start playing music on my laptop. An Apple support person helped me resolve the issue by disabling the macOS feature that responds to the "Play" button (F8) key. Apparently, some bluetooth devices send a "play" signal on hangup (according to them, more likely the OS missinterpreting it since it feels random and happens with my AirPods, Uproar BT headset and Willfull wireless headset). From the terminal, they had me disable the "Play" button as follows:

1. Open terminal
2. Issue the command: launchctl stop com.apple.rcd
3. Issue the command: launchctl unload /System/Library/LaunchAgents/com.apple.rcd.plist

While this doesn't fix the problem under iOS for those suffering from that, it did fix it under macOS (since others in this thread have reported it there too.)
