I have 2 ordered and received 2 unlocked 32 GB iPhone 4s. One was ordered on 11-11-2011 and had a SIM ejector tool. The other was ordered on 12-6-2011 and did not have a SIM ejector tool in the box.

I was afraid that the 2nd phone would not be unlocked. This MacRumors thread makes me feel comfortable assuming the 2nd phone is actually unlocked.

I'll know this weekend when I fly overseas and try a micro SIM from another county and network provider.
 
I bought both the AT&T full price device only 4S (no tool) and the official unlocked shipped from Apple. Inside packaging is different between the two phones I got.

I didn't watch the video (on celly net, atm), however, if he got the iphone 2g later on (say, after Apple folded and allowed carrier subsidy), then the later models may not of ever gotten a SIM slot eject tool, especially in the USA, where it was locked, in almost all ways effectively, to the AT&T network.
 
I don't think my AT&T 4S came with a SIM ejector either, and I also don't have paperclips in my apartment because my household is largely "paperless" - this isn't even something I did intentionally, as much as something that just happened "naturally" over time.

I ended up having to use a coaxial cable I had lying around as a SIM ejector. I'm not sure I would necessarily recommend doing it this way, but the pin in the center of the cable worked pretty well for me. :)
 
I found another tool that gets my sim card out.
