I don't usually have problems with my card, but I have seen unambiguous "read error" and "funds unavailable" messages (as well as "incorrect PIN" and the oh-so-helpful "declined"). The other day at McDonald's I got "MAC error" and a second attempt worked. I took a look at my online banking when I got home and found that I had been debited for the failed transaction, credited it back, then debited a second time for the working one. It's good to know that the system will credit you back if there's a problem :)
 
this is an old thread but this might be helpful to someone...

A few days ago I wanted to buy $600 worth of computer parts online. (I'd never spent more than $300 in one purchase on that card before). The transaction was rejected; I tried again in case I mistyped something. It failed again, so I called the 800 number on the card. After typing in my information, the automated system asked me if I had tried to make the transactions. I hit 1 for Yes, tried my order again, and it worked... no need to wait on hold to talk to someone.
