The iPhone 4 "issue" was real. I know because I did have the iPhone 4, and simply touching the "spot" reduced the signal. And of course, as shown by Apple, this issue existed on other phones as well. Now, in many places around the world, this was not such a big deal since presumably the wireless carriers have enough signal strength to begin with. The issue became apparent on AT&T in the US, where in certain part of the countries, it didn't have strong enough coverage/signal. Times the number of iPhone users in the US, and of course you have people noticing and complaining.
CDMA iPhone 4 was released later, and the antenna design was fixed, similar to the 4S design.

Eh, I used my 4 and 4S caseless and they never broke on me even. 6+ is actually the first one I've used a case with since it's just too damn slipperyI know fully that the issue was there (and shouldn't have been) but because the iPhone 4 was kinda delicate with its glass surfaces and by modern standards a very small phone, adding a cheap bumper or case did no harm and offered better protection.
If you actually understand technology and look beyond the user facing "features," Apple did significant improvements on every iteration of the Ax SoC. The Tick-tock (number and S) cycle is designed to match the 2-year typical phone contracts.I don't know whether Apple is intentionally rolling out new features slowly as a way to put consumers in a position to buy, buy, buy over and over and over again or whether they're just plateauing in terms of technological innovations,
I will say, though, I think the implementation of 3D touch is probably the most innovative thing Apple has included in their phones since Siri on the 4S or maybe the transition from iOS 6 to iOS 7
Good thing I hate and never use cases.I've decided to jump to "s" versions because of the premium case availability. By the time the S models arrive the premium cases are refined and available.
