It has to do with how OSX treats your iPhone. On Windows it shows up as a camera. Browse into the file system and drag and drop what you want to keep. As far as I can tell the only option for this in OSX is using import software such as iPhoto. It works, but why? Well, I don't use my Mac to store photos, that's for sure.
Use spotlight to search your Mac for "Image Capture."
Works exactly the same as Windows Explorer.

Thanks for that tip! Why doesn't this automatically load? This is one of those things I thing almost nobody knows about!
My one complaint is the layout. There is a relatively large thumbnail of each picture making the number of files I see per page limited. Not a deal breaker though. Before today I didn't even know I could do this.

From what i know photo stream uploads all of your images to the cloud, right? I dont need that, i only needed to transfer a pic to my mac on the move, just to show something, i didnt want to connect the cable :/
Just open it with the phone connected and select "When this device is connected open..." and select "Image Capture." It doesn't open automatically because people kept complaining about programs opening automatically when the phone was connected, the manual is helpful here.

What does that even mean? There's like ten different ways you can do it. You could say that about any manufacturer; there is always going to be a finite number of ways to do something.
It means Apple's way of doing thins is restricted, nothing new here.
You're restricted to e-mail, iTunes wired transfer, iTunes wireless transfer, Bluetooth transfer (using an app), photo stream, DropBox (or any other file hosting app), or connecting your phone to your computer as a camera.
How many ways do you actually need? There comes a time to say enough is enough; that's enough ways to do something, let's bloody move on.

There are at least 5 ways to transfer a file, none complicated:
1. Email (your phone doesn't have email?)
2. Photo stream
3. Sync with cable
4. Image Capture app
5. Sync over WiFi
None of which work with two people who wish to exchange pictures but do not wish to share their personal details, such as e-mail address (parents at a soccer game would be one example).
