Like C DM said, if you are financing the phone through Verizon then you have the equipment fee. This is why in my analysis of the cost of Apple's program makes this equipment cost "0", but adds an "Apple" cost.

I can't say exactly, but when I talked to Apple support, they told me that the credit checking for Apple's upgrade program is done with the Verizon account. So if you already have a Verizon (or other major carrier I would assume), you can join the program provided your credit is in decent/good standing.
