I also have volume issues.

I've noticed this a lot when listening to podcasts, and it seems that using the "30 second rewind" feature will cause the volume to go back up. Very odd, but seems to almost assuredly be software related.
 
i have the same problem but only when i'm on a phone call... all other volumes are fine even the speaker phone while on call. this is so weird! :\ i have to plugin headphones when im on a phone call. :(
 
I just experienced this today - specifically with the speaker volume. Trying to make a phone call, I couldn't hear a thing, just barely audible with the volume button cranked all the way up. Same deal with saved messages. The ringtone volume plays fine, the clicking to turn it on is fine, music volume is fine, I tried a game and it was low at first but I was able to bring it up. Messages sound fine on the speakerphone as well. The only thing that seems borked is standard phone volume. I tried rebooting and flipping the mute switch but no luck.
 
What I've noticed is if I skip forward/back a track the volume suddenly drops significantly. However, if I pause/unpause or move the scrubber a bit, the volume jumps right back up.

Anyone seen this issue? (Yes, the EQ is off, sound check is off, and even the sound limiter is off.)
I'm having this exact same issue and it's driving me freaking nuts. Pause/skip will change the volume high to low and vice/versa. Same settings as you.
 
Saw another thread that mentioned that the problem is occuring when there are too many apps running in the background. In my limited testing, after closing all running background apps, the sound fluctuation problem is gone.
 
Same problem here

I can confirm that I have the same problem here. Sister has the exact same iPod touch (3g) running ios3 and has twice the output from internal speaker.
 
I am absolutely embarrassed to have found out that my sound issue was simply that the ear speaker (up top) was covered by the new protective shield I had on it. In other words there was no issue at all.
 
simple restart

I had the volume issue where I could barely hear the phone including videos, music, everything. A simple restart of holding the home button and the on/off button down until the logo reappears did the job for me, give it a shot or just restore the phone in itunes.
