Do you use gmail or yahoo or live?
None of the above. Using Exchange.

And do you use the mail app that came with your iPhone?
Yup.

I'm trying to find a good primary email to use with my iPhone 4.
Define "good". It's a highly subjective word and polling others isn't the way to find out what's best suited to you. Polling only tells you what's popular. What do you need/want? What are your criteria? Do you have any experience with any of the options you're considering or additional information that might help people to give you recommendations suited to you?

The options you've listed are all free. Create accounts on all of them and see if you like the web interface and other features if they matter to you.

Which email services allows you to get instant notifications when you receive an email?
The closest to instant that I've seen would be exchange. My work email used to show up on my phone before it would show up on my desktop. I'm not sure which email providers offer that currently as I just use iCloud which is good enough for my personal email.
