As of right now there's no way they could fit a decent camera in it. People are whining about the 480p camera on the rMacbook and that camera is still larger than anything in the  watch. There's zero space. Or you could ask for a smaller battery.

You put it in a separate unit which you strap to the other wrist so you look symmetrical

If the camera is for Facetime, it doesn't need to be amazing. It just needs to be okay. And it will happen, maybe not in gen 2, but I bet by gen 3 it will be included.
