I sold an att phone. It was activated in store but never unboxed. Sold it and the buyer put it his SIM. It DID NOT ask for my account info.
 
Apple.com and picked up in store. I'm almost positive when I went to the att store right after to get my 6 back active that had something do to with it.... although you may disagree.
 
Another data point for a fully paid up front phone bought from the Apple store online. I bought an X from Apple.com for use on Sprint. I've already activated it and am using it. I got a GSX report on it to confirm the unlock status and noticed something interesting. The original lock policy on the phone was Sprint Locked. The current policy is Multi-mode Unlock.

So when the phone is shipped and unactivated, it comes locked to the carrier it was purchased on (Verizon might not be). After the first activation, though, it becomes unlocked -- as reported by someone else on this thread (not sure who, I'll update to give proper credit if I find the post). The phones won't be unlocked until they've communicated with Apple's servers. So just as WolfSnap said in 1.2.1, you need to go through the first activation step to get phones ordered from Apple.com unlocked.
