Here is a thought, and not sure why it’s never been implemented, but we have OLED displays on these newer iPhones. Why can’t they just have small notifications like flashing circles or even icons that have notifications? They could take up a minuscule amount of pixels, do the pixel shift thing, and since the phone needs to only illuminate those specific pixels, it really doesn’t seem like it would use much battery. Not to mention the phone could amp the brightness of said pixels up to a higher level. Is this not a pretty solid solution?

For Always On Displays to really be competitive there is bit of tech involved. Using the Apple Watch 5-6 for example, they use a display tech (low temp poly something oxide) that is capable of dropping down to around 1 frame per second refresh rate. For the display to get that low of a frame rate it needs a dedicated driver that can handle it. Plus I believe there is ultra low powered hardware specifically for the functionality and more sophisticated light sensor for whatever type of algorithm they feel is best for dimming the screen or whatever.

An iPhone that gets an always on display be using similar tech likely improved upon.
