I'm talking about after a year and the phone messes up. Will I be eligible for the full price I paid?
Probably not. Amex will probably want a repair estimate from authorized apple repair shop (or apple store). They will want to you get at the best a $269 out of warranty replacement and will reimburse you for that amount.
You will not get the full $650. They reserve the right to give you the lesser of the cost ($269) if that "fixes" your out of warranty problems.
And a replacement iPhone will fix your problems.

The rear camera on my iPhone 5 broke (hardware failure confirmed my apple store). The replacement cost for the phone through apple was $269 as my phone was out of its one year warranty. The original cost of the 64 gig phone was around $430 including tax.
American Express extends many manufacture warranties for the same period of time, after the originals warranty expires. I call Amex and give them all of the information regarding this case, and they credit my statement for the FULL $430. I explained to them that the replacement cost is $269 but all they cared about was the original purchase price.
Don't forget about Amex extended warranty people!!
Yeah Amex changed their warranty. I just did out of warranty repair. Expected to have to fax them my out of warranty exchange for $269 plus taxes. They didn't even want it.
They refunded me the $299 (32gb subsidized iPhone 5) plus taxes.
