Update: Moved my iPad to 18.1 and it had access immediately, which seemed a sign of a problem!Nope, I just checked again, and no go. It says I am on the waitlist.

Ultimately had to erase all content and settings and restore from iCloud backup, but everything is working on the phone now.
In case anyone else tries to solve a similar issue, here's a list of things I tried before the erase and restore that did NOT work:
- Restarting the phone
- Changing region to an unsupported region, restarting phone, changing region back, restarting again
- Resetting network settings
- Resetting all setting

Took us four years to get Watch ECG function in Australia due to stupid government requirements.Makes sense. You can't complain that a particular piece of tech is violating some part of the DMA if it's not available in said region to begin with.
And before people complain that Apple is being vindictive, I would also like to point out that living in Asia, numerous Apple features and services are not available in my region either, such as news, fitness+, Apple Card, sports, and I only recently got Siri support for my tv remote.
