I’m in Canada with a US purchased watch. ECG works on 5.1.3 but I’m afraid to update to 5.2 in case Apple threw in a nasty surprise like yanking the ecg app for those not physically in the Us.

I wouldn’t worry. They won’t do that. I did same (USA aw4)
 
The Numbers on the Explorer Watch face are missing, as in the Betas of watchOS 5.2.
Anyone with the same problem?

update just installed and I get the tutorial on the health map and it says to open the ekg app on my phone but its not there (germany)
 
Alternate headline: Apple Watch series 4 finally goes on sale in Europe and HK.

This is the feature upgrade that will most probably make me buy one!

I can't be sure but I think you'd be OK. Apple needs approval in each country to sell the watch. Once you buy it you should be able to use it anywhere. It wouldn't make sense any other way.
 
If someone is using the function outside of the USA, with a model purchased in the USA, when updating, will this function be lost if it is not in the countries that were activated?

Discrimination of Canadian Apple Watch 4’s was based on some “Canadian” serial number type thing, not your iTunes region, not your location gps either.
 
In the preview of the Watch face in the Watch App, the numbers are still there. They where missing since Beta 3.
Gotcha. I think it’s more likely they overlooked updating the preview rather than the numbers somehow disappeared from the actual face unintentionally (and wasn’t noticed/fixed before the 5.2 final). But of course it could be that the numbers not showing on the actual face is a bug.

There was talk of Apple using the SIM card to approve/deny ecg. That might apply to new setups and not existing installations.

Time to update.
