Make it yourself, or have someone else make one for you? I know plenty of people who can screenprint shirts, and I've done iron-ons before (but going the iron-on route sucks. Just find someone with a screenprinter and ask them to do it for you.) Or there's probably no shortage of online services that can do this for you for a small fee.
I'm not sure about this. I like how they wear brighter colors so they are easier to see. This is helpful in the local Apple Store which is usually pretty crowded.
I worked at Apple retail, circa 2001-2003. Black shirts were very common then. Biggest problem was that black tends to: 1) fade as it's washed 2) show lots of hair and dust (if you have a cat or dog, you're screwed). They also used to be in more of a stretch material, not just a cotton t-shirt.

Sometimes they end up on Ebay after the fact. That's how I got my Apple hockey jersey from the OS X launch in Canada.

Surprised they aren't black shirts. Most of my clients are in the high-end salon hair products and almost all of them wear black. Black (dark) clothing is the typical style for high end fashion stores so I guess Apple is splitting the difference with friendly t-shirts/polos in the intimidating fashion color.
