I had seldom cases when calls were working fine after carplay, mostly when I waited for the complete shutdown of the car before jumping out (usually I park and immediately get out).
After a ride few hours ago it was also fine, but now tested again and issue is present.

So weird. I have an aftermarket radio, so I don’t have a situation where it shuts down differently depending on how quick I exit.

Regardless, hopefully they fix it fast!
 
Still having the same issue, also BMW. Has anyone tried Resetting All Settings on the iPhone in case the bug has carried through? Considering doing it.

There are people who upgraded to iPhone 12 and still have the issue, so reset will not help unless you are ready to try restore as new.

I'm from Spain, and having the same issue with my Apple Watch Series 4. I have a Kenwood radio with Wireless Carplay. When I restart the phone, the problem is solved until I connect again. Talked with Apple Support, but no luck.

I thought I was crazy when I was having this issue. So I restarted my Apple Watch and iPhone XS then sync the watch and phone again. It work when I made some calls but later in the day it stopped working. I won't get calls from my watch but I can make calls through Siri on Apple Watch.
 
My watch doesn't ring either after using VW Golf's wireless carplay. 14.3 b3 didn't bring any change unfortunatley.

Same problem here. While I was on the phone with Apple Support I found this forum. I was able to work around the issue by turning off CarPlay (2018 BMW). Now my watch shows the incoming call notification. Then tried it with CarPlay back "on". It still works. I suspect that next time I connect to my car I will lose the incoming call notification to the watch. Bug report has been filed.
 
Currently this is the most annoying bug going on with iOS. An extremely close second is messages being auto-read and not getting notified. Both need to be fixed by Apple ASAP.
 
This was driving me insane and i would have never thought it would be carplay. Looking at this thread, this has been an ongoing issue for some time now. How has apple not fixed this yet??

I thought I was having an issue with this also, as ive noticed missing a few calls lately when wearing the watch and having the phone on vibrate....watch never alerted me. I noticed it for sure yesterday when I saw my phone ringing and the watch wasn't so I came here to see if others were having the same issue. I also use CarPlay for driving to work, but never would've associated the two as a common denominator.
 
I’m beginning to wonder if they even know. My initial bug report (albeit confusing and before I knew it was CarPlay related) was closed as “works as intended.” My new bug report says zero similar reports. Frustrating.

My bug report is still open, as my previous two reports for macos, dated ..... 2015.
There are contradicting updates on other forums:

Guys I read a work around earlier from another user (cannot remember who) - I have a BMW 4 series and iDrive 6 - once I switch the car off (press the Off button once), I unlock my phone, then open control centre, then hit the wifi toggle. It asks me if I want to disconnect from CarPlay. I say yes. Then I press the Engine off button again to fully shut down the car (For some it is opening the door that fully shuts off the car).

My watch has been ringing every time since. An extra step that we didn’t have to do previously, but at least a work around until Apple fixes the issue
 
Same issue for me.
Using a VW with wireless CarPlay.
Only iPhone reboot will solve it.

What a mess!
 
Same issue here to. Apple watch s5 and iPhone 12 pro. Misses calls all the time because my phone is always silent.
I am considering 14.4b1 to see if it is fixed.
 
If somebody try iOS 14.4 beta, please comment. Thanks

EDIT: Just discover a new bug related whit this. After disconnect CarPlay, can't use the walkie talkie... when somebody try to send me a Walkie Talkie message, I only receive a Facetime audio (null) call on my iPhone.

Was so hopeful the updates would fix it... When the watch update didn’t I still held hope for the phone update... but sadly it didn’t resolve the issue either!! 😞 ...I don’t think I’ve ever had such a fundamental issue with an Apple product in all my years using them!!! Please fix this soon Apple 🙏🏼🙏🏼🙏🏼
