I believe Apple only did that on US carrier phones, since they already have their respective SIMs preinstalled. It might also be true in some other markets, but majority of iPhones worldwide that are SIM free have the SIM ejector still included in the box.

My 11 Pro Max was a Verizon phone and had the tool.
 
The original iPhone is the only one that ever came with a cleaning cloth.

I always buy my micro fiber cloths from the eyeglass store. It's very coincidental as I just purchased a new pack of two today.

I've been doing this since the original iPhone, as the one Apple included was just ok.

Same. Since the 3G until now I’ve skipped only 3 models. The 5, the 7 and the X. I’ve not seen any cleaning clothes in any of the models I bought and I always bought directly from Apple.
Original iPhone came with one. None since then as far as I can recall.

They did it here in the uk also.

Spent the first hour with 11 pro max desperately looking for anything that would fit before finding a sewing needle.
It's in the little "Designed by Apple in California" folder (at least it was in my UK 11 Pro).
