A simple gesture based deal would do wonders here. Not sure why Apple has held off with gestures on the iPhone. It's one of the tweaks I cannot live without.

I agree with you there. WebOS gestures were the best...
I agree that being able to reach every point on the screen isn't necessary, but I'm still surprised with your ability to easily reach so much of the screen (do you have superhuman dexterity?). I'll concede that you're the one actually holding the phone though, so you'd know better than I would. I just don't think I'd prefer a screen that large.

I think that a gesture would imply that 2 fingers are involved, in this case it's no longer a 1hand operation.
Dolpin Web browser you can draw something like a < and it will take you back a screen. You can easily do that with your thumb

Yes but that would add a layer of complication to iOS in itself. One could argue that shapes could've been used in Mac OSX as opposed to the swiping gesture using different # of fingers, but I think it's just simpler in implementation and faster (debatable if faster depending on the symbol).
Ya the discussion was about one handed use. The Galaxy Nexus has an elegant solution to the unreachable back button - the back button on the bottom of the screen which works for pretty much every app.

Apple has made it a point in their design to value obviousness above all else. A gesture is not obvious.

iPad has a home button, back buttons, etc.
You can operate OS X without gestures if you wish. ( dashboard button, mission control button, etc.) I think it's the F keys (F3, F4, etc)
They have never made gestures essential to the operation of their products. And they won't b/c gestures aren't obvious.
Although the obviousness principle applies more to the post pc devices. Everyone knows how to use PCs now.

Well then why has Apple been so insistent on 4:3 for iPhone AND iPad so far? Why is 16:9 all of a sudden better?
