Roughly since I upgraded to iOS 10, I have increasingly noticed the instability of how iOS interprets Apple Pencil input. This is one of the issues that is extremely frustrating.

Quite often, unwanted line segments are generated between two separate strokes drawn by the user. Please see the attached screenshots. I have not found specific conditions to reproduce this, but it happens both in single-app-mode and multitasking mode, both when writing slowly and writing quickly. I normally rest my palm on the screen while writing.

It happens across all kinds of note-taking apps that I use on my iPad Pro 12.9 (I also got a chance to try these apps on iPad Pro 9.7 and can reproduce the issues as well):

It happens so often (can be up to multiple times per minute) that it has reduced my productivity drastically. As much as I wanted to convert to paperless lifestyle, this is one huge issue of Apple Pencil that's preventing me from doing that.

Anyone experiencing the same issue?

(I have filed a bug report to Apple. I am using latest iOS and all app versions.)

Does this occur when charging? I've had issues with my 12.9" when charging from my 5 port Anker (which outputs ~10W per port). I don't experience this when charging from Apple chargers though.

It does happen to me whether or not I'm charging the device (and when I charge I use OEM Lightning cable).

Is the tip screwed on tightly? I've seen things like this when the tip of the pencil was not completely seated.

I just figured out. It's the Zoom in Accessibility. When you turn off Zoom, these issues disappear completely. I think it has to do with conflicting three-tap gesture events and the drawing events sent by Apple Pencil.

Was very frustrated for several months over this bug.

Turn off the 5-finger gestures.

Sorry for the late reply.

I noticed this on non-Apple stylii on the iPad and it (usually) turned out to be an issue with gestures, especially when resting the hand on or near the display.

I have to say I have yet to see it with the Apple Pencil, but I have multi gestures turned off.

I've noticed a quirk in procreate where short strokes are getting deleted immediately after drawing them. Anyone know of any weird settings that could be causing that? It seems to happen at random, but frequent enough to be frustrating. iOS 9 still (but only started recently) and 9.7 pro.
