Thank you for great observations.A few more observations playing around with this in the last few days
1. The "From" Aliases do appear on older iOS and macOS. It just takes a bit of time to show up. You don't need iOS15 or the beta iCloud page
2. Removing and then adding an email in your domain doesn't make that email show up in "beta.icloud.com" from or aliases, but shows up on iPhones and macOS and you can send emails from these IDs. --> Reported via Feedback assistant
3. If your Apple ID was your custom domain email and then you add that email in custom domains, too, you cannot send emails from this address --> Reported via Feedback assistant
4. If you're forced to make an "@icloud.com" email just to get this to work, you can easily disable it from bothering you. Just go to "https://beta.icloud.com/mail/"-->Settings-->Composing-->Send From-->Uncheck the iCloud email

I changed my Apple ID to an @outlook.com email and then removed and re-added this alias again. Now it works fine. I have reported this in Feedback assistant.The odd thing is this was working at first, but then randomly broke so they changed something.

For me on iOS and macOS, the email addresses appear raw, without the name. I don't mind, because when the recipient receives my email, they see my name.Btw. are you able to send email from macOS with your name and not just with email adress? From web client, iOS and iPadOS it works fine, but from macOS I'm still sending only "raw" email address.
