Did you migrate data to a new iPhone from an older one? This happened to me, and the only thing I found that fixed it was to turn off iCloud Photos on the phone, delete all the locally saved (full size) photos, then turn iCloud Photos back on and let it redownload the 'optimized' versions.
I used iCloud to restore. Thank you for the suggestion!
 
does it fix the Intermittent Touch Issues/ screen wont light up after touch issue?
It does not, hopefully iOS 15.1 will fix this. I did read that 15.1 beta 2 had a fix for this but I cannot confirm that.

I've never seen this before where it isn't available in iTunes. I prefer to update over iTunes even though it might not make much of a difference. What would the reason for this be?
 
Good to know but it may still not be available on itunes. Another source doesn't show it either.

IPSW is being signed now so it should show up on iTunes

Can you confirm that if you do this (turn off iCloud Photos on the phone, delete all the locally saved photos, and re-enable) won't delete anything from iCloud Photos? Won't it somehow "cache" the fact that I deleted the photos while it was off, and then upload the changes to iCloud?

While I'm at it, my impression has always been that updating via iTunes actually swaps out the entire OS for the new one, whereas the OTA updates only patch it. Is this still true? Or does it really not matter which method you use?

Yes, OTA is a patch while IPSW is the full firmware. I’ve always went with IPSW regardless what version it is. I’ve experienced less issues with IPSW than OTA.
 
Under the current build is it better to set up as Direct Transfer or iCloud restore? I saw someone reference they were getting full size photos on direct transfer - is that a known issue?

You will definitely want to make sure your library is fully synced before turning it off, but once you've turned iCloud Photos on a device, any changes you make on that device will not be synced to the cloud — I had Photos open on my MacBook Air and did a couple of small batch deletions on my iPhone after turning off cloud syncing as a test, just in case, but nothing changed in the cloud version of the library.

I did a direct transfer, and it did indeed transfer the full-quality versions of most items in my iCloud Photos library; not sure how widespread this behavior is, though (1 or 2 reports does not a trend make).
