I suspect the same as you… However, others here are not reporting the same issue?
They are, not every canon raw file is an issue but everyone with a 5d mark iv has this issue I’ve tried on different machines as well
 
Adding to the chorus: Quick Look is not working as it should with .CR2s (from a 7D) on a MBP running 15.4. Argh!!!
- Hitting space bar reveals black Preview.
- Resizing Preview reveals photo.
- Old thumbnails display photos.
- New thumbnails do not display photos.
Have filed a bug report with Apple. Using Adobe Bridge until fix is offered.
Why are steps forward often accompanied by leaps backward?

PS - Have tried these steps, but they didn't lead to a remedy.
Restart your Mac
Clear Quick Look Cache
Stop Quick Look’s process and restart it
Force Quit Finder
Relaunch the Finder app
Check the app’s settings
Restart the Mac in Safe Mode

I wonder if we can see a consistency with the CR2 and bodies that are failing to display properly? I posted earlier not having issues with an M2 but my old CR2 files came from a 350D that I haven't used in a decade ++.
 
I literally went today to buy a new MacBook Pro just to go through my photos for a wedding I did and delete the ones I don’t need, then copied them to a hard drive and put them on my other machine, ridiculous. Gonna return it obviously. Not gonna lie, I don’t really like Space Black, my m1 mac space grey looks nicer lol the Space Black is a fingerprint magnet haha
 
I have the problem as described on my Mac Studio. I expect they'll fix it; odd that resizing works.

Meanwhile I use Fast Raw Viewer. Faster, and a better viewer. For culling etc etc. it's a better tool, and as of now I don't mind just double clicking on the occasional R5 raw to get a view. But I don't spend much time in Finder ever browsing raw images, and I feel the pain of those of you who have to instead of using other software.

Also having this issue with macOS 15.4, with CR2 files from a EOS 5D Mark IV. Seems it still has not been fixed, and also seems like it's been an ongoing issue for awhile.

It has ruined my work flow I had to buy another Mac just to do my work and I’ll return it. Apple should have fixed it by now. I have reported it

This is blocking my MacBook Air from opening CR3 files from Canon EOS R1.
I FTP the images from my camera to a folder on my Mac using "FTP Server app" from the App Store. all worked well until the Sequoia 15.4 update and now all the RAW images are flagged as potential malware from application "FTP Server". Need to manually approve each file in Privacy and Security system prefs to open them individually.

The CR2 downloaded from https://www.imaging-resource.com/PRODS/canon-70d/YIMG_0069.CR2.HTM opens correctly with space in Finder and displays correctly in Icon view & Gallery view. In Column view appears black, but I'm testing in a virtual machine.
Used that sample. That is exactly what I get on my Intel iMac. M4 MBP fails with both space (Quick Look) and column view. Same as my CR2s.
Edit: after adjusting the size of the Finder window, the image appeared in Column view as well.
So it does! On both my Macs.
 
Same here with Nikon stuff.
I tried different NEFs coming from old cameras as D2x till D810, D4 and D5. Only black rectangles in preview.
I found some very old Canon files - CR2 coming from 5D Mk I which have correct previews.
My computer is MBP M1 Max.
 
I’m out now, but now macOS 15.4.1 is out has this issue been fixed, I doubt it but let’s hope
