UPDATE!: Finally got my phone.
Just some background for those asking, I had the original package held at the ups customer center. They gave me an empty box with the return label in it. I called Verizon and the supervisor told me that they mailed me just the trade in kit and that the phone will come separately (spoiler alert, she was wrong).
I ended up having to sort things out with Verizon on the phone for 4 hours. Nobody could understand that the iPhone was stolen and what to do next. They told me that they would open an investigation and that it should hear back in about a WEEK

After getting nowhere with customer service, I emailed the executive office customer service email. I got an amazing executive assistant that sorted everything out for me and had my phone sent out next day, even though it was back ordered 2 weeks at this point. He gave me his email and his direct extension if I needed anything else, and waived the upgrade fee. He was fantastic and made everything so easy (I won’t give his name out for his privacy).
It’s a shame I had to email the executive office to get this sorted out, but I’m glad this fiasco is finally over.
