Do y'all think we will get the app being on the latest watch beta, or will we have to update to 5.1.2 public release?
I think you will be offered the public release. I don't think Apple has anyway of hiding features in a given build and then unhiding them remotely.

Does EKG (ECG) work outside of the US with your regional settings set to US?
Yes, it works, im in Guatemala and I have it.

Which settings did you all have to change?
I just have the regional settings on US, my Apple ID is from Guatemala.
What you can try, is to use a VPN, turn off the iPhone and Watch, the turn it on, put the VPN on US, turn on the Apple Watch and search for the icon on the watch.
* I didn't use VPN, just installed the update and the app was there.
** I did purchase the watch on the US.
 
I have the ECG app on the Apple Watch 4, but nothing comes up in the HEALTH app on the phone. Phone is on 12.1.1

Start the ECG app on the Apple Watch and it should prompt you to setup and continue in the Health app on the iPhone. Setup the iPhone Health app and it should all start working.

Tried that, isn't making anything in health pop up.

I think the phone is on 12.1.1. beta.. is that an issue. Seems like it shouldn't be. I've tried rebooting everything several times.

Yeah, you need to be on the public release and not the beta.
Also, your iCloud account must be in the US region.

The key, per published reports, is where you acquired your Watch. If you bought it in the US, you will be able to use the ECG app now. If not, you can’t.

No amount of playing with regions, either in your phone or your iTunes account will change this. This is very locked down, and for a good reason: if Apple allowed such easy methods of gaining access to this in countries where it has not been approved, Apple could face fines, lawsuits and could even be barred from selling the Watch or even the iPhone.

Don’t be angry at Apple about this: it’s just following the law. Ask your governments to fast track approval if you want this soon.

Yep, removing the beta profile and installing the current release of 12.1.1 (I was on 12.1.1 but it was a beta channel release) did the trick and the ECG app wizard started right up after reboot. Bad news is, apparently I'm dead.
 
Does anyone else now have their Watch stuck on full brightness?
(5.1.2 on Series 3)
The only way I don’t get blinded is keeping it in Theatre mode and slowly turning the crown.
This happened straight after this update.
