I have an Apple Watch S4 bought in Thailand. My Apple ID and all Apple devices including iPhone are set to France (region and language).

It does not show ECG or EKG.

In order, to make it available, I should be in the country that the features are available in or not?
What is the criteria to see this features.

It's based on country of purchase, which means it will depend on the model number.

My Watch S4 and iPhone XR were bought from HK. Currently I'm in mainland China, I'm trying to setup the ECG it shows that the region is not supported. The region settings is set to HK as well. I think it depends in your actual region. The software detects your region regardless of which region is selected on settings.

Did you buy them from Apple or a third party seller?

I think you have to connect to carrier in Hong Kong for initial setup. I’m sure if you travel to Hong Kong you’ll be able to do it.
