* I don't have a problem in normal use (even as a lefty), but can make the signal go away entirely using the death grip. An interesting test that I was using was to get the SpeedTest.net app and give it the death grip while running the test. I'm able to get the latency / download / upload to start and stop at will by applying and removing my hand.
Reception issue: Yes, if I use the death grip I can completely make my reception go away I can't even get a reading using speed test unfortunately. (fortunately, I haven't been affected very much by the issue in normal use.)
Yellow screen: nope and as far as I can tell everything else is perfect on my phone.
If it's a hardware issue like the PA, then the lot # is probably the most important information since the PA are sold in lots. Certain lot #s could have a high failure rate, while others could be perfect. The only way to tell is crack open your iPhone. This is in regards to the reception issue.
I think we can give up on the when and focus more on the where. The first two digits of the serial number indicate where it was manufactured. Only a hand full have posted this information so it makes it difficult to know if the "issue" is related to build location.

Reception Problem: The # of bars displayed seems very erratic compared to my old phone, regardless of the way I'm holding it. Not sure yet if this actually translates into dropped calls or poorer call quality.
