Says in first paragraph that Apple will be the exclusive provider.

My question is about the servicing. Since this is Assurant Insurance. Looks like Apple still does everything

Thank you for buying AppleCare+ with Theft and Loss, an insurance coverage underwritten by American Security Insurance Company (“ASIC”) with its administrative office at 11222 Quail Roost Drive, Miami, FL 33157-6596 (305.253.2244). Apple has agreed with ASIC to be the exclusive provider of all services and replacement devices to you under AppleCare+ with Theft and Loss.

yeah I saw that also when I started doing more research. I wasn't going to say anything because I wanted to see if I could get more details about exactly what happens. but for the moment I'm guessing that you file a claim, they vet it and then report back to Apple that it's approved for a swap. or they might even has some kind of access to the apple system to be able to order replacement phones from apple, enter it in the record etc.

True, but this could get tricky. After my phone was stolen a few months ago, my first call was to AT&T to block the IMEI. Doing so cut it off from the network entirely. Plus if the phone is turned off after theft, how would Apple know whether Find My iPhone was active?

it's in the system. iCloud reports back that it's on when you flip the switch. if you turn if off from the phone it reports that also. if the power dies you can remove it from the iCloud site or the find my iPhone app

I disagree. For most people the iPhone Xr will be fine. And the Series 4 has ECG that required FDA approval. Quite amazing technology.
Apple has the ability through iCloud settings to verify FMP is on and last backup.

I already got that AppleCare for my iPhone X’s. It’s worth It for a thousand dollar phone. This is good for if you payed full price or monthly apple finance. If you don’t have carrier insurance then this is an option.
