I saw Account # and last 4 of SSN, but what was the other info needed?

I'm eligable for an upgrade in december (Netherlands). However insurance just upgraded my broken 1.5 year old 3G 16g to a brand new 3Gs 16g, so I'm good for now. Will skip iPhone4 and upgrade on iPhone5.
