Still waiting. Has anyone been turned down? Anyone other than GoreVidal have success?
No in the USA Verizon uses CDMA for the iPhone 4S. If you have an AT&T iPhone 4S and unlock it then you can use and compatible GSM carrier or reseller but not CDMA.If I unlock my iPhone 4S, would a Verizon SIM card work in it (like from an iPad 3)?

But the hardware is universal, right? The Qualcomm chip supports both GSM and CDMA, right? So there is an additional lock at another layer?

Crap well AT&T turned me down for the unlock request via their site. Seems like the reports of people in-contract getting theirs unlocked via the site is indeed a crapshoot. I'm going to call them later just to see if maybe I get a rep who is willing to do it, but both myself and my wife (both still under contract on our 4S's) were turned down.
I'm able to upgrade with the extra $250 plus fee, my wife has to pay full retail for ip5.
Is this the same message you're getting?
"We cannot complete your request because we found no valid current or former mobile or account number for this request.
If you have questions, review the general eligibility requirements for unlocking an iPhone."
I can't tell if that's saying they won't unlock because I'm still in contract, or they are having some problem and don't "see" any line on my account that's an iPhone...

Yes I got the same message... I didn't want to waste time resubmitting so I just paid the $8 or whatever it is to the seller unlock_fusion on eBay and an hour later I have a working, unlocked 4S still under contract up on sale on eBay.
If anyone's interested in either a 64GB White or 64GB Black, unlocked GSM iPhone 4S, my eBay seller ID is james_lepage. Phones are in awesome condition with AppleCare+ until 10/2013.
