It is on all the time, but only for certain apps. I am usually in areas where there is WiFi and I use that.
No, I have unlimited and while vzw just raised my ratesand I don't "abuse" it, I turn off when home to save battery life, such as it is.

Have you checked your WiFi assist setting in relation to dropping the WiFi connection?Im turning it off whenever i dont use it or need it on coz i dont have unlimited data. also i noticed in the house when the wifi signal dropping a bit (but still usable) its 'jump back' to cellular, even if its not really necessary. and lets not talk about system services eating cellular data even if i dont use them...

Depends on what you have running and using data and not just the switch itself. The switch just enables a method of getting data. It does not request data.Does just leaving cellular data 'on' use up any data?

If you are on WiFi, you aren't using mobile data anyways. Turning it off when you are on WiFi does nothing for battery life savings, as it's not being used in the first place. If you wanted to save battery, you could toggle airplane mode though.
