I try to use the same one. But if someone has taken my name (yes it has happened) i will change it by one number 🙂

And i always keep my password the same as my username Just Kidding 😛

My last name is Manzanilla, which, if literally translated from Spanish, would be apple-ish or apple-like. But actually it means chamomile.
 
I've used CaptMurdock on almost every forum since the late nineties. It's actually derived from a character in a Star Trek RPG I had for many years.
 
The only place that's really different is here at MacRumors.

Frankly I'd like to change my username here to my normal one, but I don't want to lose my join date and whatnot... 😱

No.

In the case of MacRumors all the names that I could think of were already taken. That evening I had a wonderful sushi dinner, so I put sushi in on a lark and it worked. 😱

But I do enjoy sushi so I guess it's okay. 🙂

Mostly the same.

I've used and maflynn (first and middle initial plus my last name) for what is eons, it actually predates the internet, in that its my work id that I used to log into the mainframe when I used to work on mainframes back in the day of Cobol and such. Lately I've found that in some sites, that's already been taken so I opt for either mflynn or either name with a number at the end of it.
 
Yes, but my passwords vary alot and in complexity.

I try to do that, but some sites limit your passwords. They don't allow special characters or limit the character length and it's bloody annoying. My bank only allows a max of 8 characters. My passwords usually start at 8 characters!

I don't use the same username everywhere on the Internet.
But I do tend to use some of my usernames on more than one site, usually sites that share a similar theme. For example, I've just created my Apple Game Centre account with the same username as my PlayStation Network ID.

This username is one I made up.
I'll typically use fragments of phrases or names I read in fiction or see on favourite TV shows, whatever has inspired me for whatever reason around the time of creating an account...

I think I quite like the concept of one username everywhere (it's tidier), but at the same time i don't like some of the security implications or losing the advantages of compartmentalisation.

If you go through the proper channels (i.e. pm a mod) that wouldn't happen. I was abiyng when I first joined in October of 2007. My handle is a portmanteau of my full name, so when I got married, and my last name changed to Jenkins instead of Young, I had my handle changed to abijnk. Everything else on my account stayed the same.
 
My current username three used to be my internet wide name. I switched to using skyrocket or skyrkt as my new internet wide name though.
