No problem. Normally at least under iOS 6, Voice Roaming had to be on to complete activation. Maybe that is changed now and your phone was jumping to Sprint and couldn't complete activation. Anyhow, glad to see I was able to help.

I have an AT&T iPhone 4S which I want to use for this. How can I switch over? I am currently still paying on AT&T but am out of my contract so I can cancel anytime. I think there is a way to unlock the phone, if I am able to unlock the phone, do I just start up a chat with page plus and that's it?

AT&T iPhone is GSM so it doesn't have a MEID. Good news, since you've got a GSM phone, you don't really need to jump through hoops to switch to a different carrier. Just swap the SIM card and you're good to go. There are plenty of AT&T MVNO's which don't require you to unlock your phone. You only need to unlock if going with T-Mobile, etc.
Since you're out of contract, you can request AT&T to unlock your iPhone officially:

Unfortunately, I am not aware of any easy way to flash a Sprint 4s. I am sure there is a way but I have not heard of data working then...
The phone is GSM so Page Plus will not work. You are looking for an AT&T MVNO like Red Pocket, AirVoice, AiO, or H2O. If unlocked you can use T-Mobile.

This a Page Plus guide so please excuse me if my advice is incorrect. However, since US Cellular is now an official carrier, in the latest firmwares there should be a US Cellular bundle? There should be a PRL that you can install into the existing carrier bundle. I may be wrong though.

There is a Page Plus carrier bundle pack that you can use on Cydia. However, I do not think that it will work as the 5c is an LTE device. Coupled with the CommCenter patch, it may work...

I have used pageplus for years (4 lines) but have always used droid devices. My daughter now wants to try the iphone 4s. Both my kids have ipods but we have no other i-devices so I'm a little lost.
First, some auctions on ebay for the 4s say something along the lines of "can only be activated on the verizon network"...is this true or can it still be activated on pageplus-using your guide of course!
Second, I have read that visual vm may not work, but other than that, everything else should function correctly right? Does it matter what ios the phone we purchase is running for best compatability. Does the 4s use wifi ok? We use the 1200 plan on PP and it works for us because we keep data off at home and use wifi.
Also, should we do anything different to do esn change rather than a new or ported number? I have had success with doing chat previously, but wanted to make sure there weren't any further instructions and if necessary we are not against using an outside activation source.
Lastly, should she be able to transfer her music from the ipod to her new iphone without much difficulty?

Page Plus uses the Verizon network so it will work beautifully. You must ensure it doesn't have a bad ESN. Most sellers will clearly dictate that it does if it has one. Simply swap the ESN numbers over and dial *22890. The 4S natively works with Page Plus, just as the same with the iPhone 4.
Visual Voicemail will not work, however notifications and classic Voicemail *86 will work fine! Music can be transferred right over either through iCloud if purchased on the iTunes Store, or via iTunes. Wi-Fi will also work fine.
Back in January I got my iPhone 4 activated. Today I got my iPhone 4s activated with my number I had on the 4, and the 4 got a new number which was passed down to my sister.
Was pretty seamless and couldn't be happier! When the chat person moved over my number to the new phone, I did get my remaining minutes and such moved over as well. She did say they couldn't activate 4G phones but I said please and she did it and all works!

For some reason Page Plus sees the 4S as a 4G device. It is not but I believe it has a conflicting ESN range with other 4G devices.
The CDMA portion of a 4S is strictly a 3G device. It does not support HSPA + like it does on AT&T. Glad you got it working though.
I found that weird too... I'm just bummed that they can't activate any of the iPhone 5 series, but whatever the 4S works fine enough.
It is because Verizon (the network PP uses) says MVNOs can't use 4G devices to reserve it for their own prepaid plans. The Straight Talk iPhone 5 works as it has 3G disabled. It is close to the perfect prepaid iPhone as it has unlocked GSM, pure 3G CDMA, and the ability to go 4G LTE when PP allows.

That's good to see that you got that working! I never was the jail breaking type but I am glad to see their contributions.
I just unlocked my AT&T iPhone 4S and tried to activate on pageplus but it seems they only accept CDMA phones not GSM phones (AT&T) so didn't work for me. Is there a work around for this or do I need to buy a CDMA phone?
As mentioned in my original post, you must be using a CDMA iPhone 4/4S that is for Verizon. GSM is a completely different technology.

I want to switch over BUT don't want to loose my data - contacts, videos, apps & much, much more than I have icloud storage or computer storage for (lost my 2 computers recently & not replacing yet). So, 2 basic questions (maybe you'll see more):
1) Will you reprint your guide to doing this?
2) How can I save all my data going to PP?

In summary, iCloud backs up your app data, Photo Stream, settings, and documents and data. It does not backup all apps and music so those don't count towards your free 5 GB plan! I highly reccomend running a backup before attempting this.
I would say to go to MyVerizon and get an idea on what the data usage was for the past couple of months. On the small end, Page Plus's lowest plan has 10 MB which is used by a couple of iMessages. On the high end, there is a 5 GB plan. You need to decide what your needs are, and if you need the unlimited or are close to it, it may benefit you to keep Verizon.
Provided you are out of contract, simply contact Page Plus and port the number. They will walk you through this and give them the ESN. Your iPhone will continue to work throughout until the port completes where you will receive an email. Then go to Settings --> General --> Reset --> Reset Network Settings and simply dial *22890 and you are migrated over.
It is a simple process. I do recommend verifying with Verizon what you use on your plan, how much you pay, and that you are out of contract. If it is close to the same price, keep Verizon's unlimited!

Will check the PP rates again. I thought they still had unlimited everything but it's been a while since I checked. I kept hearing I had to flash my 4s even though its on Verizon & several folks said I could mess everything up (& no way to recover it) since it's a 4s rather than a 4.
I still don't get (understand) an easy way to back it all up. Maybe I'm just slow...
I have 40 of icloud but at least 62 stored on phone. Maybe buying more storage space in icloud via itunes it backup everything temporarily will store it all while I port over to PP?

I don't understand what you mean by I have 40 of iCloud. Either way, you can easily backup to iTunes by simply connecting your device and selecting Backup Now.
There is no flashing as Page Plus natively supports Verizon 3G devices. It is the same as activating the phone with Verizon.
What are you running Google's data center off of your phone's connection
. In all seriousness, hold onto that Verizon plan for as long as you can. Then go to T-Mobile.
