My replacement was hand inspected by the looks of things. Before I even took it out of the box I noticed fingerprints all over it. And yes, it was a sealed phone.
Nope, don't have Applecare. I should mention, this is in the UK. Might be different in the US.

Did they send you a new phone? My replacement is coming directly from China. I wish it was the white box phones. I feel like there is a better chance for it arriving without dings or dents.

If you don't have apple care and they send you a replacement via Fedex or UPS they will charge you $29 unless you get them to waive the fee.

Wow . . . . how in the world could that pass any stringent QA tests?

For those getting replacements in the UK when order online straight from Apple, does your replacement arrive as new in a sealed iPhone box just like you'd bought it new?
I've just got my replacement ordered. Have to say Apple UK were fantastic about it, wasn't on the phone 10 mins and they had approved it. It should be here in 3-5 days and they said to arrange a date for pickup of my old one for after the new one arrives so I won't be without a phone. Ideal!
But after reading some of this forum I was wondering if it'll be like new, sealed in the box with all accessories?

Got my replacement black yesterday, and it's much better than my original one.
There are some faints scuffs (I hesitate to call them that) around the rounded corners on the top-left and bottom-left, but they are so minor they hardly bother me. There is a light scuff on the border of the anodized aluminum that is hardly visible, but again, that doesn't bother me.
I've got an iPhone 5 I feel is good enough to keep. You might think that's compromise, but after going through replacement after replacement on my iPad 2 for backlight bleed, sometimes good enough...is good enough. You could wind up with worse.
I'm not quite ready to call this one flawless, but it's near-flawless, and due to the nature of the mass-market manufacturing process - it's, yes, good enough.
