It is safe to update to the newest version (by using the sierra_patcher integrated "on install update tool") ?
If yes, then via manual combo update or App Store ?

You guys should have installed either Foxlet's patch or CZO's patch (the latter via dosdude1's latest post installer tool).
If you have one of those installed (and are pointed at the production catalog), then you should be able to grab the 10.12.4 release via the Mac App Store.

Installing on my 2008 17" MBP at the moment...

And for those (like myself) that are still doing it manually - the distribution package is 091-03392.

Fischersd - Thank you for your fast reply ! As you mentioned in your answer, i have installed OS with latest dosdude's patcher (so with latest post installer) cca 1week ago. Till this time i was able to update iTunes absolutely "normal" as on supported machine. I didn't change anything in patcher or installed some other "update enablers" because the whole system works fantastic, as i told = absolutely original. In this case, i should have all everything ok and pointed to the right "adresses", and i should update via App Store ? I know, it is a dumb question, but since 15years with mac's i never made "unsupported" installation...

If you have downloaded the Combo updater you have to modify the distribution file of the .dmg.
There is an explanation on how to do this on page 151 of this discussion.

On my MBP4,1 everything works fine. Backlight (on this modell) still requires the old AmbientLightSensor plugin into AppleSMCLMU. 10.12.4 still not removes my patch for softwareupdated. Night Shift works after patching CoreBacklight.

If you haven't been installing any of the beta releases, you never would have changed from the production catalog. If you applied the patch via the latest post-install utility (CZO's patch), then you should see the 10.12.4 update become available via Mac App Store.

btw...if you want to alert someone's attention you need to put an @, then their user name - so they get an alert, so they know you've asked them a question.

Those patches are designed to allow updates to be downloaded and installed from the App Store. I would recommend using Czo's patch with the App Store. No reason to install it using the PKG. If you absolutely must for whatever reason, you have to modify the Distribution file inside the PKG to bypass the system check. If you need to install Czo's patch, you can download an installer script from here: http://dosdude1.com/sierra/swupatch.sh.zip

Confirmed that MacPro 3,1 early 2008, installed 10.12.3 without any beta 10.12.4, can go ahead and update 10.12.4 through App Store.

Can you tell what changes this patch make to the system? If a want to remove it, how would be?
 
It downloads an SUVMMFaker.dylib and replaces the com.apple.softwareupdated.plist. Just simply 'fixing' the system check for future updates.
Thanks! I made a backup of softwareupdated.plist just in case.

Also, 10.12.4 working fine on my MacBook Pro 5,5, installed with App Store using czo patch.
 
Updated MacBookPro5,1 (Late 2008) to 10.12.4 via AppStore - no problems

Is there anyway to obtain the 10.12.4 DMG file from the app store on a non supported MAC?

If not does anyone have a link to the Full DMG file uploaded somewhere I can have.

There is no "full" Disk Image for macOS Sierra, you can get the App Store bundle (if you're licensed) using tools such as DownloadSierra.
