Is it possible to pair a watch on WatchOS 3 with an iPhone running iOS 11, or does the watch also have to be updated to WatchOS 4?

It's fine. You just can't pair Watch OS 4 with iOS 10.

Yes you can. Just got my watch back from a downgrade of watchOS 4 but still running iOS 11.

I live in country no have apple store. I'm not sure how long it take to downgrade OS.
