I did mine at the end of September and just received my shipping materials on Friday. I wouldn't worry if I were you, they'll come.
Well they offered me $300 for my iPhone 5s 16GB Silver, and its better than what I was offered by Gazelle. Thats why I decided to trade it in with Verizon. I traded in my parents old iPhone 4s' with Verizon and didn't have an issue but, I don't remember having to wait so long for mailing materials. As long as I get them by the time my phone ships, I'm good.

I was offered $300; 200$ for phone +100$ promo. You think if I take the paper/confirmation that you get when you request a quote a retail location would honor it?
Ordered sg 16gb plus on 10/12/14. Walked into my local Walmart today and just for fun asked if they had any 6pluses. Well what do you know they had what I was looking for. I quickly called VZW to cancel my order and the rep told me my order was already cancelled!! I didn't do it, so tomorrow they are going hear my opinion. But tonight I'm busy setting up my new phone##
I called on Thursday to ask about the return envelope and was told it will be in the box with my new phone. I was skeptical (I still am, actually) but I'm not sure what else I can do except wait. My phone will be here on Tuesday.
I would really prefer to just do the trade in at the store but all the reps there I've asked said it had to be mailed in. I might try it anyway.

This morning I ordered the phone with an estimated ship date of 10/31

You did. I mentioned that every Chicago Apple Store had every model 6 Plus in stock, which they did for all of Friday, and that I tried canceling my order to pick one up, but verizon said no. So I'm still stuck waiting for my online order.I looked at the spreadsheet earlier today and thought i saw that you found one in store somewhere....

Earlier I had the CSR fill out the online rebate form in the store, and once I had my 6+ I brought my 4s to he store. The CSR looked up my account and asked me about whether I had reset the phone (I had), wrote some numbers down and gave me a $200 credit. I could have also gotten the gift card sent to me. Then he gave me a receipt for the payment.
i havent seen any on flightaware tonight. but there should be some tomorrow and the rest of the week for sure...what model and when did you order?
yeah i hope my 6 plus 64gb SG gets shipped by friday..ive been waiting since 9/17..longest month of my life
According to this part of the email I received when I got my appraisal, you have until no later than 12/01/2014, but they request 7 days after receiving your new phone.

I submitted for the rebate thing on 9/28 and also just got my envelop from Verizon a couple days ago. They are taking longer than the two weeks to get them out.
I am hoping one of the local stores will take my old phone and give me the credit as others have had luck with. I do not need the $200 gift card right away, but just do not trust the mail in process because of things I have read.
I do know if you are going to mail it, take pictures/video of the condition of your old phone, that you have reset it and made sure the "find my Iphone" feature is turned off.
