I've lived all over the US in rural areas, and the telephone part of the iPhone has always been pitch perfect for me.
Then I visited New York for a few days. Now I see what all the complaining is about.
It's the phone's fault, it's the network. The phone works flawlessly when you're in an area with good coverage and not too many iphone users. But in New York, there are just too many people using the service at the same time and the phone part works horribly as a result.
But you can't really fault either Apple or the iPhone for that.
