For the record, this never worked with an already activated sim card. You always needed a brand new, never used, never before activated sim card. Most people have good luck just asking for a new sim card if they tell the rep what they want (ie tell them you need a new sim to activate through iTunes with your iPhone). Short of that, you can always buy an unactivated sim. Standard sims on ebay are under $2 and micro sims are under $5, as of this writing, of course.
I went into a local store the other day and asked for a sim card for activating the iphone 2g but they would would not give me an inactivated sim card instead they would try to give me an already activated sim card. That could be just my local att store. I am really hoping it is decided whether this works or not anymore soon
