Not even in the UK, it was Ireland. This is O2 UK and its only in the UK and not the US
I have the exact same problem, also on T-Mobile NLHelp!
After fooling around trying to get MMS to work I am now getting the following error:
Thu Mar 19 17:04:57 unknown CommCenter[33] <Notice>: Error (61) activating PDP context 0 that supports types
0: kGprsPdpAuthenticationFailure
(opening Safari, etc it gives the error Could not activate cellular data network: PDP authentication failure. I *do* have a 3G icon)
My 3G won't connect anymore on my iPhone, I restored the original ipcc file and also reset the network settings through Settings -> Reset.
I'm on T-Mobile NL, is anyone else having problems getting data service? Or did I possibly get myself flagged at T-Mobile because of failed attempts to connect or something??
I have the exact same problem, also on T-Mobile NL

And it's also not possible to login anymore;

This is how I have achieved tethering with iphone via USB.
PLEASE MAKE SURE YOU HAVE ENABLED TETHERING WITH A CUSTOM IPCC
*more info on that here - http://twitter.com/timmeh/status/1351833678 and here - http://www.tuaw.com/2009/03/19/dept-of-that-was-awfully-quick-eager-devs-enable-tethering-o/
• Connect 3.0 iphone to mac via USB.
• Enable tethering via USB.
• In network click the add service button (little button with a plus bottom of the list) prefs
• Choose the ethernet connection of your iphone (often the highest en, on my machine (imac) it was en4)
You should now be able to browse the net tethered to you iphone.
T-Mobile NL Network is up again
Edit:
This is how to make USB Tethering working, not taking credits...
