The only issue was they brought out a matte black instead of the jet black - but honestly that worked out to my benefit as I like the matte better than the jet black.
Since my 6 was case bound since day 1 it had no scratches on it and the screen was perfect condition so according to them it qualified for the $649.99. It's an all/nothing deal on it though, no pro-rating based upon damage. The only other option is doing the trade deal where you only get up to around (I think it was last time I checked) $50-$225 (depending on device).
No, and that's not what the process is. You must purchase the phone on their installment plan. Then after 3 months they start crediting it back to you on the installment plan with a monthly pro-rated amount. When getting it from Sam's, you are basically getting it from Verizon. They are an authorized reseller. No different than going into one of those small authorized reseller stores that say Verizon on the front that aren't actually a corporate store but private owned.I know this has been answered - but the iPhone 7 must be purchased from Verizon directly to be eligible?

I just mailed mine off yesterday, so hopefully it'll be there by Monday/Tuesday.

To figure the savings, just divide 649.99 by 24 (it's something like 27.08). That's what you will be credited back for each month.Did they mention the payments schedule?

It shows that it will take 2-3 billing cycles... and from what I understood they will also credit you for the 2-3 months that you had to wait until they started (so if it's $27 per month and it takes 3 months then you will get a lump sum of $81 then the remainder at $27 per month).
