I JUST GOT MY UP TO DATE CODE! The code worked in the app store and mountain lion shows up correctly in my purchases, and the installer is downloading.
I submitted the up-to-date online form about 1 hour ago.
I gave my serial number and address. I didn't give my phone number. I also didn't have to upload any receipts since I bought mine at Apple.com's refurbished section.
If you have 10.5.8 you have to get to 10.6.8 first by using a snow leopard DVD. Then you can jump straight to ML.
So far so good! Tested all my apps here on ML and all are working fine, every bit of installed applications remained intact. Seems more like an incremental upgrade though, as many have mentioned in this forum, but for the price it's worth to leave your machine up to date!

I just resubmitted my claim this time it said serial number already claimed . but i pushed okay a few times and then it went to another page to ask for a copy of the purchase. so will see what happens

Same here! I entered my info and got the claim reference number on my screen, but still no e-mail from them and it's been 4 hours!
my update went very smoothly, been running ML for a few hours how.
received my claim code within minutes of submitting, and the download only took about 15 minutes

Installed on my 15" MBP 5,1. Initial impression is an increased processor load compared to Lion. Let's see if this remains the case.

Is anyone running ML on a Air? Does it bring back the "instant-on" feature (computer wakes up instantly similarly to iOS)? While it was still advertised in Lion, it was definitely not instant compared to Snow Leopard or iOS...
My download status bar doesn't look like it has moved since it started and it's been 30 mins it would be cool if they would have gave a est. of the time remaining.
