Just used the treadmill and did 3.33mi with the Apple Watch app. No badge. Suffered through it with a hangover to get that badge too! Glad I did it but really wanted to earn the special achievement.

It seems that you have to use the built in Workout app in order to get credit which is not what the description would have you to believe.
I set up "indoor walk" and the goal as 3.30 miles. I haven't yet calibrated it but I doubt that would be the issue.Did you actually set up an exercise or you just did it on your own?

Well, I just completed my run in the Nike running club app. And I did not get the achievement. Mine was over 4 miles. It's showing up in health as well as the Activity app as a run. But I didn't get the achievement. Anyone help!
So after running 2 5K's trying to get the stupid badge using a 3rd party app, I just did an outdoor walk using the Workout app and it finally gave me credit for it.

I did 4M today and didn't get the badge. Then I realized the watch thought I was in Aruba (so, my bad for not setting the correct region).
After I set the region to USA the outline of the award appeared, but it's not filled in. Don't know if it will rectify itself, but I have to take the blame if it doesn't. Oh well, the run still counts as far as health benefits, I can live without the pat on the back.
Wow! nice output today! I ran a 5k first thing this morning and didn't get the badge. I had planned to walk a 5k after dinner, but my stomach ain't having it.
Why don't you try a hard reset?
I find it odd that you are able to see the award outline, registered > 5k, but still not get it. Maybe because it needs to be there *before* you register the run for it to recognize your workout?
Let us know if it works in case you choose to reset.
